UNIT OWNERS MAINTENANCE REQUEST FORM

 

 

UNIT OWNER

Use this form to report problems that require Maintenance. Complete the information listed below and submit to the Hutchinson House Office. This form can also be used for requests / suggestions to improve Hutchinson House Grounds and Facilities.

OFFICE

Make two (2) copies of this form and distribute as follows:

bulletMaster- File in Office Open Items Building Maintenance Book.
bulletCopy 1- Give to Vice President HH for follow up on action.

 

HH Manager:

Take action on discrepancy as soon as possible. If the discrepancy is an item that needs HH Board Approval contact the Board for approval.

Set priority on the item and establish a completion date. Record the Completion Date on the Master with any comments as to status.

HH Board Vice President

Review all maintenance discrepancy forms submitted with the Board President and follow up that job action is being taken and the Master Form is being updated. Check completion dates to make sure work is on schedule and report to the Board the status of all projects. Forms with suggestions must be submitted directly to the Board for action by the Vice President!
